Who We Serve:
Arise Care Services (ACS) proudly supports adults with intellectual and developmental disabilities (IDD) who seek community engagement, life skills development, and social participation. We accept participants under the Virginia Medicaid Waiver (DD/CL/FIS) and also welcome private pay clients.
Enrollment Steps:
1. Referral or Inquiry — Submit via phone, email, or online form.
2. Intake & Assessment — We learn each individual’s goals and needs.
3. Personalized Plan — We develop a tailored support plan.
4. Start of Services — A DSP is assigned, and services begin promptly.

- Current Individual Support Plan (ISP) and service authorization
- Emergency contact and health information
- Completed consent forms and transportation agreement
